Empath Health is seeking a compassionate PRN Home Health Speech Language Pathologist to provide individualized care in patients’ homes across north Pinellas County, including Clearwater, Palm Harbor, Tarpon Springs, and surrounding areas.

Perform initial and ongoing assessments, develop a care plan with the physician and team, and guide patients on swallowing safety and communication goals. Florida SLP license and 1 year of SLP experience required; home health experience preferred.

Qualifications

• Graduate of an approved/accredited program meeting ASHA requirements for certification as a Speech-Language Pathologist;CCC-SLP or in process of certification.
• Active Florida SLP license
• Minimum of 1 year of experience as an SLP; home health care experience preferred

Responsabilités

• Perform initial and ongoing SLP assessments (including goals for swallowing and communication).
• Develop care plans in consultation with physicians and care team.
• Instruct patients and families on swallowing safety, nutrition, and adaptive strategies.
• Document services and progress notes in a timely manner.
• Collaborate with the interdisciplinary team to ensure holistic home-based care.
