Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health SLP to deliver individualized care to patients in their homes in Naples, FL and surrounding areas.

Responsibilities include assessments (OASIS when applicable), care planning with the physician and team, and educating patients and families on swallowing and communication strategies. Join a mission-driven team providing Full Life Care for All.

Qualifications

• CCC-SLP certification or in the process of certification with CCC requirements.
• Active Florida Speech-Language Pathology license.
• Minimum 1 year of licensed SLP experience; home health experience preferred.

Responsibilities

• Perform initial and ongoing assessments, including OASIS, to determine swallowing and communication status.
• Develop care plans in consultation with physicians and the care team.
• Educate patients and families on swallowing safety and nutrition changes.
• Instruct patients on strategies to support language, cognition, and sensory-motor development.
• Prepare timely progress notes and service documentation.
• Collaborate with the interdisciplinary team for holistic home-based care.

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes in Naples, FL and surrounding areas.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations

Home health visits available in Naples, Bonita Springs, Immokalee, and/or Marco Island. It is very flexible - you choose when & where you want to work! Schedule: PRN role; 5-10+ home health visits per week. Part‑time roles may be available as we continue to grow!

What you’ll Do

• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
• Develop a plan of care in consultation with the physician and other care team members.
• Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
• Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
• Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What you’ll Need

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
• Active Florida SLP license
• Minimum of 1 year of experience as a licensed Speech Language Pathologist (SLP); Home health care experience preferred
• Strong clinical assessment, communication, and patient‑education skills
