Job summary

Empath Home Health, part of Empath Health, is seeking a compassionate PRN Home Health Registered Nurse to provide in-home care across north Pinellas County, including Clearwater and nearby communities.

The RN will perform comprehensive assessments, develop care plans, deliver skilled nursing services, educate patients and families, and collaborate with the care team. Weekend/holiday on-call rotations are required, with growth potential to full-time as demand grows.

Qualifications

• Must be a graduate of an approved nursing program with active Florida RN license.
• BSN or MSN degree preferred.
• Minimum 1 year of direct patient care experience; prior home health RN experience strongly preferred.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in north Pinellas county (including Clearwater, Palm Harbor, Tarpon Springs, and surrounding areas).

Locations: Home health visits in north Pinellas county (including Clearwater, Tarpon Springs, Safety Harbor, Palm Harbor, Dunedin , and surrounding areas).

Schedule: 10+ home health visits per week. Required weekend/holiday on-call rotations.

What You’ll Need

• Graduate of an approved nursing program with an active Florida RN license
