Job summary

Outcome HC is seeking a full-time Speech-Language Pathologist for Shorepointe Care Center in New Jersey to help residents regain communication abilities and swallowing function, enhancing overall quality of life.

You will work within an interdisciplinary rehabilitation team, create personalized treatment plans, document progress in the EMR, participate in care plan meetings and family conferences, and supervise SLP assistants when needed.

Qualifications

• Active SLP license in NJ or eligible to obtain one.
• New grads welcomed with mentorship and training.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Evaluate and treat residents with speech, language, cognition, and swallowing disorders.
• Develop individualized treatment plans to improve communication and dysphagia management.
• Collaborate with physicians, nurses, PTs, OTs, and the rehabilitation team.
• Document progress in the EMR accurately and promptly.
• Participate in care plan meetings and family conferences.
• Supervise SLP Assistants as needed.
• Report to the Director of Rehabilitation.

About the Role

We are seeking a passionate and dedicated Full-Time Speech-Language Pathologist to join our dynamic rehabilitation team in our skilled nursing facility in New Jersey. What You’ll Do

• Evaluate and treat residents in a subacute and long-term care setting with a variety of communication, cognitive, and swallowing disorders
• Develop personalized treatment plans to improve speech, language, voice, cognition, and dysphagia management
• Collaborate closely with physicians, nurses, physical therapists, occupational therapists, and the interdisciplinary team
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Speech-Language Pathology Assistants (if applicable) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Speech-Language Pathologist license in the state of New Jersey (or eligible to obtain)

• Previous experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team

• Certificate of Clinical Competence (CCC-SLP) preferred, but Clinical Fellows (CF) are welcome to apply
