Job summary

San Simeon by the Sound Center for Nursing & Rehabilitation is seeking a full-time Speech Language Pathologist/CF for a Skilled Nursing Facility. The SLP/CF will work with adults with complex communication needs in a collaborative rehab team and SNF setting.

Responsibilities

• Conduct comprehensive assessments and evidence-based treatment of speech, language, cognitive and swallowing abilities.
• Develop individualized treatment plans tailored to each patient’s needs.
• Collaborate with families, educators, and healthcare professionals to support patient progress.
• Complete accurate and timely documentation including daily notes, progress reports, evaluations, and discharge summaries.
• Stay current with advancements in speech therapy practices and medical terminology.
• Ensure compliance with facility protocols and state/federal regulations.
