Crystal Lake Healthcare & Rehabilitation in Bayville, NJ 08721 is seeking a dedicated Speech Language Pathologist (SLP) to join our rehabilitation team. We offer Full-Time or PRN positions with a resident-centered environment and supportive leadership.

You will assess, diagnose, and treat speech, language, cognitive, and swallowing disorders in adult and geriatric residents within our skilled nursing facility.

Crystal Lake Healthcare & Rehabilitation is seeking a dedicated Speech Language Pathologist (SLP) to join our growing rehabilitation team at our Bayville, NJ 08721 location. This is an excellent opportunity to work in a supportive, resident-centered environment under a highly experienced leadership team, including our new Administrator, Director of Rehabilitation, and Director of Nursing.

Position Summary

As a Speech Language Pathologist, you will assess, diagnose, and treat speech, language, cognitive, and swallowing disorders in adult and geriatric residents within our skilled nursing facility. You will be an essential member of our interdisciplinary team, helping residents improve communication, safety, and overall quality of life.

Key Responsibilities

• Conduct comprehensive patient evaluations
• Develop individualized treatment plans aligned with resident goals
• Provide speech therapy using evidence‑based interventions
• Document all assessments, progress notes, and therapy outcomes
• Collaborate with nursing, rehab, families, and physicians on care plansAdjust treatment plans based on progress and clinical judgment

Requirements

• Degree in Speech‑Language Pathology from an accredited program
• Active New Jersey SLP license
• Strong clinical assessment and treatment skills
• Solid understanding of speech/language anatomy and physiology
• Excellent communication and documentation abilities
• Long‑term care experience preferred, but not required
