Job summary

Career Links is seeking a full-time Speech Language Pathologist for a skilled nursing facility setting. This role offers variety, professional growth, and the chance to impact residents’ lives across the continuum of care.

Responsibilities include evaluating and treating residents with speech, language, cognitive-communication, voice, and swallowing disorders, developing individualized plans, and collaborating with an interdisciplinary team.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • CCC-SLP certification preferred.
  • Experience in a SNF or geriatric setting preferred, but new graduates are welcome to apply.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Evaluate and treat residents with speech, language, cognitive-communication, voice, and swallowing disorders.
  • Develop and implement individualized treatment plans based on physician orders and resident goals.
  • Perform dysphagia assessments and provide safe swallowing interventions.
  • Document evaluations, progress notes, and discharge summaries in a timely and accurate manner.
  • Collaborate with interdisciplinary team members including nursing, OT, PT, physicians, and dietitians.
  • Educate residents, families, and caregivers on treatment plans and progress.
  • Ensure compliance with state, federal, and facility regulations.
  • Participate in care plan meetings and team conferences as needed.
