Job summary

Coordinated Therapy Solutions is seeking a Speech Language Pathologist to evaluate, diagnose, and treat patients with speech, language, cognitive, or swallowing disorders in SNF settings.

You will collaborate under physician direction, develop individualized care plans, administer assessments, document accurately, and educate residents and families on practice techniques; state licensure required.

Qualifications

• Master's or Doctorate in Speech-Language Pathology from an accredited program.
• State license as a Speech-Language Pathologist required.
• Proficiency with applicable state, federal, and professional standards.

Responsibilities

• Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.
• Under a physician’s direction, provide direct patient/resident treatment based on an established plan of care.
• Screen and evaluate residents and develop appropriate care plans.
• Plan, direct, or conduct rehabilitative treatment programs to restore communicative efficiency.
• Administer, score, and interpret specialized hearing and speech tests.
• Instruct residents, families, and caregivers to monitor speech and practice new skills.
• Complete required forms and document in accordance with policy and regulations.
• Maintain patient records accurately and timely.
• Devise special tools and treatments for patient needs.

Job Summary:

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.

Duties/Responsibilities:

• Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.
• Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.
• Screens and evaluates Resident and develops appropriate care plan
• Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
• Administers, scores, and interprets specialized hearing and speech tests.
• Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.
• Maintains patient records in an accurate and timely manner.
• Devises special tools and treatments for the specific needs of patients.

Education and Experience:

• Graduate of an accredited Masters, or Doctorate in Speech language Pathology.
• State license as a speech pathologist required.

Physical Requirements:

• Prolonged periods of sitting at a desk and working on a computer.
