Job summary

White Oak Healthcare is seeking a Speech Language Pathologist to evaluate, diagnose, and treat patients with speech or language disorders, and to formulate individualized treatment programs.

You will work under physician direction, screen and evaluate residents, and plan rehabilitative treatment to restore communicative efficiency while documenting care and maintaining patient records.

Qualifications

  • Must hold state license for Speech-Language Pathology.
  • MS or PhD in Speech-Language Pathology.
  • Familiarity with applicable state, federal and professional standards.

Responsibilities

  • Evaluate, diagnose, and treat patients with speech, language, cognitive, or swallowing issues.
  • Under physician direction, provide direct patient care based on established plan of care.
  • Screen and evaluate residents and develop appropriate care plans.
  • Plan and conduct rehabilitative treatment programs to restore communicative function.
  • Administer, score, and interpret specialized hearing and speech tests.
  • Instruct residents, families, and caregivers to monitor speech and provide practice strategies.
  • Complete required forms and documents per company policy and regulations.
  • Maintain accurate patient records in a timely manner.
  • Develop tools and treatments tailored to individual patient needs.

Job Summary:

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.

Duties/Responsibilities:
  • Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.
  • Under a physician's direction, provides direct patient / resident treatment based on established plan of care.
  • Screens and evaluates Resident and develops appropriate care plan
  • Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
  • Administers, scores, and interprets specialized hearing and speech tests.
  • Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.
  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.
  • Maintains patient records in an accurate and timely manner.
  • Devises special tools and treatments for the specific needs of patients.
Required Skills/Abilities:
  • Knowledge and skills to care for patient needs of the SNF population
  • Must possess problem solving skills to successfully perform job
  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Strong customer service orientation.
  • Must be able to communicate effectively and professionally with other employees, patients, and family members
  • Ability to travel as needed to perform job
Education and Experience:
  • Graduate of an accredited Masters, or Doctorate in Speech language Pathology.
  • State license as a speech pathologist required.
  • Proficiency with all applicable state, federal, and professional standards
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer
