Job summary

Coordinated Therapy Solutions is seeking a Speech Language Pathologist to evaluate, diagnose, and treat patients with speech or language disorders within a SNF setting.

You will formulate individualized treatment programs, maintain records, and educate families on monitoring and practicing new skills. A state license is required, with opportunities for professional growth and direct patient care responsibilities.

Qualifications

  • Graduates with accredited master’s or doctoral degree in speech-language pathology.
  • State license as a speech-language pathologist is required.
  • Knowledge of applicable professional standards and regulations.

Responsibilities

  • Evaluate, diagnose, and treat patients with speech, language, cognitive, or swallowing issues.
  • Under physician direction, provide direct patient treatment per care plan.
  • Screens and develops appropriate care plans for residents.
  • Plan and conduct rehabilitative treatment to restore communicative efficiency.
  • Administers, scores, and interprets hearing and speech tests.
  • Instructs residents, families, and caregivers to practice new skills.
  • Completes forms and documents per company policy and regulations.
  • Maintains patient records accurately and timely.
  • Devises special tools and treatments for patient needs.

Job Summary:

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.

Duties/Responsibilities:

  • Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.

  • Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.

  • Screens and evaluates Resident and develops appropriate care plan

  • Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.

  • Administers, scores, and interprets specialized hearing and speech tests.

  • Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.

  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.

  • Maintains patient records in an accurate and timely manner.

  • Devises special tools and treatments for the specific needs of patients.

Required Skills/Abilities:

  • Knowledge and skills to care for patient needs of the SNF population

  • Must possess problem solving skills to successfully perform job

  • Excellent verbal and written communication skills.

  • Excellent organizational skills and attention to detail.

  • Strong customer service orientation.

  • Must be able to communicate effectively and professionally with other employees, patients, and family members

  • Ability to travel as needed to perform job

Education and Experience:

  • Graduate of an accredited Masters, or Doctorate in Speech language Pathology.

  • State license as a speech pathologist required.

  • Proficiency with all applicable state, federal, and professional standards

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
