Job summary

Mission Valley Post Acute in Fremont, California is seeking a compassionate Speech-Language Pathologist (SLP) to join our collaborative rehabilitation team. This full-time role offers flexible scheduling and a chance to help residents regain communication, swallowing, and cognitive abilities.

You'll evaluate and treat residents, develop individualized plans, monitor progress, educate families, and work with physicians, nurses, and dietitians to coordinate care. Compensation is $55-$65/hour DOE.

Qualifications

  • Current California Speech-Language Pathologist license in good standing.

Responsibilities

  • Perform comprehensive speech-language and swallowing evaluations
  • Develop and implement individualized treatment plans for communication, cognition, voice, and dysphagia
  • Monitor resident progress and modify treatment plans to maximize outcomes
  • Educate residents, families, and caregivers on treatment goals, home programs, communication strategies, and swallowing safety
  • Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary care team to coordinate resident care
  • Participate in care conferences, discharge planning, and rehabilitation meetings
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards
  • Provide staff education and in-service training as needed
  • Supervise CFY and student clinicians in accordance with state regulations, when applicable

What You'll Do
