Job summary

Concord Post-Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a per diem basis. This role offers flexible scheduling (up to 3 days a week, 4–6 hours per day) at our Concord, California facility, with starting pay at $75 per hour.

The SLP evaluates, develops, and implements individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing disorders, collaborating closely with physicians, nursing, and therapy

Qualifications

  • Active California Speech-Language Pathologist license.
  • Ability to evaluate and treat residents with communication and swallowing disorders.

Position Summary

The Speech-Language Pathologist (SLP) evaluates, develops, and implements individualized treatment plans for residents experiencing communication, cognitive, voice, language, and swallowing disorders. The SLP collaborates closely with physicians, nursing staff, rehabilitation professionals, residents, and families to promote optimal outcomes.

Essential Duties & Responsibilities

  • Evaluate residents and develop individualized treatment plans
  • Provide speech, language, cognitive, communication, and swallowing therapy servicesAssess and manage dysphagia and recommend appropriate dietary modifications
  • Maintain accurate and timely clinical documentation
  • Collaborate with interdisciplinary team members to coordinate resident care
  • Educate residents, families, and caregivers regarding treatment goals and progress
  • Participate in care planning meetings and discharge planning as needed
  • Ensure compliance with all regulatory, ethical, and facility standards

Qualifications

  • Current California Speech-Language Pathologist license required
