Job summary

Woburn Rehab & Nursing Center is seeking a part-time Speech-Language Pathologist (SLP) to join our resident-centered rehabilitation team. You will evaluate residents, develop individualized plans, and provide direct therapy to improve communication and swallowing functions.

Qualifications

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current state licensure as a Speech-Language Pathologist.
  • CCC-SLP certification preferred.
  • Experience in skilled nursing or geriatrics is a plus.

Responsibilities

  • Evaluate residents referred for speech-language pathology services.
  • Assess speech, language, communication, cognitive, and swallowing disorders.
  • Develop individualized treatment plans based on resident needs and goals.
  • Provide direct therapy to improve communication, cognition, and swallowing function.
  • Establish measurable, clinically appropriate treatment goals.
  • Monitor progress and modify treatment plans as needed.
  • Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time.
  • Educate residents, families, and caregivers on treatment plans and techniques.
  • Collaborate with physicians, nurses, therapists, and the interdisciplinary team.
  • Maintain complete clinical documentation; follow HIPAA and resident rights requirements.
  • Perform other duties as assigned by the Director of Rehabilitation.

Responsibilities

As a Speech-Language Pathologist (SLP), you will:

  • Evaluate residents referred for speech-language pathology services
  • Assess speech, language, communication, cognitive, and swallowing disorders
  • Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals
  • Provide direct therapy to improve communication, cognition, and swallowing function
  • Establish measurable and clinically appropriate treatment goals
  • Monitor resident progress and modify treatment plans as needed
  • Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time
  • Educate residents, families, and caregivers on treatment plans and therapeutic techniques
  • Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team
  • Maintain accurate and complete clinical documentation
  • Follow all applicable federal, state, and facility regulations, including HIPAA and resident rights requirements
  • Perform other duties as assigned by the Director of Rehabilitation
Qualifications
  • Master's degree in Speech-Language Pathology from an accredited college or university
  • Current state licensure as a Speech-Language Pathologist
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred, but not required
  • Strong communication, clinical, and interpersonal skills
  • Ability to work effectively within an interdisciplinary team
  • Commitment to compassionate, resident-centered care
