Job summary

Fitchburg Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to join our skilled nursing and rehabilitation team. The SLP will assess referred patients, develop and implement treatment plans, and document outcomes under physician guidance.

The role requires graduation from an accredited program, state license/registration, and preference for experience in long-term or geriatric care.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to evaluate medical condition, functional capabilities, limitations and rehab potential.
• Establish and administer a treatment program with goals aligned to patient capacity and physician direction.
• Set realistic and achievable goals for patients.
• Document and record patient condition.
• Educate patients and families in appropriate therapy methods.
• Complete evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborate with all disciplines for best patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Maintain confidentiality and comply with HIPAA and Residents' Rights.

Duties and Responsibilities of Speech Language Pathologist (SLP):

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
