Job summary

Woburn Rehab invites a full-time Speech-Language Pathologist to join our rehabilitation team. You will evaluate residents, assess speech, language, cognition, and swallowing disorders, and develop individualized treatment plans to enhance independence and quality of life.

You will collaborate with physicians, nurses, and therapists in a resident-centered, compassionate environment, ensuring thorough documentation and progress toward meaningful rehabilitation goals.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current state licensure as a Speech-Language Pathologist.
  • CCC-SLP certification preferred but not required.

Responsibilities

  • Evaluate residents referred for speech-language pathology services.
  • Assess speech, language, cognition, and swallowing disorders.
  • Develop individualized treatment plans and provide direct therapy.
  • Establish measurable goals and monitor progress.
  • Educate residents and families on treatment plans and techniques.
  • Collaborate with physicians, nurses, and therapy teams.
  • Maintain accurate clinical documentation.

Responsibilities

As a Speech-Language Pathologist (SLP), you will:

  • Evaluate residents referred for speech-language pathology services
  • Assess speech, language, communication, cognitive, and swallowing disorders
  • Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals
  • Provide direct therapy to improve communication, cognition, and swallowing function
  • Establish measurable and clinically appropriate treatment goals
  • Monitor resident progress and modify treatment plans as needed
  • Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time
  • Educate residents, families, and caregivers on treatment plans and therapeutic techniques
  • Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team
  • Maintain accurate and complete clinical documentation
  • Follow all applicable federal, state, and facility regulations, including HIPAA and resident rights requirements
  • Perform other duties as assigned by the Director of Rehabilitation
Qualifications
  • Master's degree in Speech-Language Pathology from an accredited college or university
  • Current state licensure as a Speech-Language Pathologist
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred, but not required
  • Strong communication, clinical, and interpersonal skills
  • Ability to work effectively within an interdisciplinary team
  • Commitment to compassionate, resident-centered care
