Job summary

Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Speech Language Pathologist to provide comprehensive speech-language pathology services in Madison, IN. The role requires active or pending SLP licensure and possibly CCC-SLP; CPR certification is preferred.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Active or pending state SLP licensure to apply for this position.
• CCC-SLP from ASHA preferred or required; CPR certification preferred.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through assessments and interviews.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication and swallowing; address articulation, language, voice, and fluency.
• Educate patients and families about disorders, therapy techniques, and strategies to support progress.
• Maintain accurate documentation of evaluations, progress notes, and plans in line with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
