Job summary

Autumn Creek Post Acute is seeking a Speech-Language Pathologist/CFY to provide direct patient care and support the therapy department. You will evaluate and treat patients, communicate with families and physicians, and document services in medical records within facility procedures.

The role requires establishing treatment plans, collaborating with the rehab team, and contributing to professional development.

Qualifications

• Knowledge in Speech-Language Pathology evaluation and treatment.
• Ability to supervise and direct assistants and aides.
• Knowledge of medical terminology.
• Excellent written and verbal communication skills.

Responsibilities

• Provides treatment programs to maximize speech, language, cognitive, and swallowing abilities.
• Develops effective plan of treatment and obtains approvals from referring physician.
• Documents services in the medical records and coordinates with families and healthcare team.
• Supervises Clinical Fellow and other SLPs in direct patient care and related activities.
• Writes comprehensive, objective progress notes and discharge summaries.
