Job summary

The Speech Language Pathologist at Mentor Hills Post Acut e provides timely evaluations within 24 hours of physician referral and develops treatment plans aligned with orders. You will supervise Speech Language Pathologist Assistants in direct patient care and communicate progress to the health team.

Qualifications

• Bachelor’s degree in Speech Language Pathology.
• Licensed as a Speech Therapist in the state.
• Masters or Doctorate in Speech Language Therapy preferred.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop treatment plans and obtain physician approval.
• Supervise Speech Language Pathologist Assistants in patient care.
• Communicate patient progress with supervisor and health team members.
• Participate in care conferences and discharge planning.
• Record evaluations and treatment notes per policy and state requirements.
• Instruct families and nursing staff for discharge preparation.
