Job summary

San Diego Post Acute Center is seeking a full-time Speech Language Pathologist to evaluate patients within 24 hours of referral and develop individualized treatment plans. The role includes supervising Speech Language Pathologist Assistants during direct patient care and ensuring documentation meets policy standards.

You will educate patients’ families and nursing staff for discharge preparation, participate in interdisciplinary conferences, and serve as a clinical preceptor for CFY/RFP students

Qualifications

• Bachelor's degree in Speech Language Pathology.
• Proficiency with Casamba and Point Click Care software.

Responsibilities

• Evaluate patients within 24 hours of referral and develop treatment plans.
• Supervise SLP Assistants in direct patient care and related activities.
• Document evaluations and treatment notes per policy.
• Instruct families and staff in discharge preparation and caregiver training.
• Participate in discharge planning and clinical rounds.
• Act as clinical preceptor for CFY/RFP SLP students.
• Maintain licensure and comply with state practice acts.
