Job summary

Meadowcreekpostacute is seeking a licensed Speech Language Pathologist to provide and direct quality patient care, including evaluation and treatment planning, goal setting, family education and documentation within our rehabilitation team.

You will supervise assistants and students, ensure patient safety, and maintain up-to-date clinical knowledge through continuing education, while documenting in EMR and rehab systems. Collaborative care with families and the broader care team is essential.

Qualifications

• Graduate of an approved curriculum in Speech-Language Pathology.
• Current, active license as Speech-Language Pathologist in good standing.
• CPR certification as required for license renewal.
• Working knowledge of Medicare and other payor sources.
• Knowledge of residents' rights and patient safety requirements.

Responsibilities

• Directs and supervises the work of assistants and students as applicable with state and federal guidelines.
• Directs all aspects of quality patient care including evaluation, treatment planning, goal setting and documentation.
• Expands clinical and professional knowledge through continuing education.
• Inputs information into EMR and rehab systems accurately.
• Communicates patient status and needs to patients, families, caregivers and other team members.
• Participates in quality assurance and customer service activities.
