Bring your clinical judgment and patient-centered approach to assess communication, cognitive-linguistic and swallowing needs, develop goals, deliver interventions, and monitor progress in a skilled nursing setting.

Qualifications

• Must have Tennessee Speech-Language Pathology licensure or be eligible to obtain licensure before practice.
• Clinical reasoning and professional judgment required.
• Strong communication and collaboration with care teams.
• Ability to assess, plan, deliver and document care within scope and credential requirements.

Responsabilités

• Assess communication, cognitive-linguistic and swallowing needs within scope.
• Develop individualized goals and treatment plans, deliver interventions, monitor progress.
• Explain plans clearly and use practical strategies for patients and caregivers.
• Collaborate with rehabilitation and care teams to align goals with daily activities.
• Document evaluations, interventions and progress accurately and promptly, and communicate changes.
