Job summary

Creative Solutions in Healthcare, Inc. seeks a full-time Speech-Language Pathologist in Houston, TX to join our in-house rehab team.

You will evaluate swallowing and cognitive conditions, develop individualized treatment plans, and educate residents and caregivers to improve outcomes. The ideal candidate holds an active Texas license in Speech-Language Pathology, understands current practices, and collaborates with the interdisciplinary team to support residents in long-term care.

Qualifications

• Active Texas license in Speech-Language Pathology
• Experience in SNF/long-term care preferred
• Knowledge of current treatment practices

Responsibilities

• Evaluate swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve independence.
• Provide skilled speech-language therapy assessments and services.
• Educate residents and caregivers; provide AAC support as needed.
• Coordinate with physicians and other professionals for diagnostic procedures.
• Ensure accurate billing records and clinical documentation.
