Job summary

Creative Solutions in Healthcare, Inc. is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care to improve swallowing and cognition outcomes.

The ideal candidate will hold an active Texas license and bring long-term care experience. You will evaluate residents, develop and implement individualized treatment plans, provide AAC support, educate families and care staff, and ensure accurate documentation and billing in compliance

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate Residents' conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments and develop individualized treatment plans
• Provide AAC systems for individuals with severe expressive/language disorders
• Provide training to clients, family, caregivers, and other professionals
• Coordinate with other professionals for diagnostic swallowing procedures
• Demonstrate competent technical skills according to current standards
• Communicate changes in resident's condition
• Provide accurate billing records and supporting documentation
• Respond to family requests and concerns timely
• Coordinate training with Restorative Nursing and other staff
• Ensure resident safety and confidentiality
• Comply with legal and professional requirements
