Job summary

Creative Solutions in Healthcare, Inc. is seeking a Full-Time Speech Language Pathologist (SLP) to join our in-house team serving residents in a long-term care setting in Texas.

The ideal candidate will assess swallowing and cognitive function, develop individualized treatment plans, and expand communication options including AAC. This role emphasizes education for residents and caregivers, with a focus on positive outcomes and resident independence.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies and boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech and language therapy assessments and develop/implement treatment plans.
• Provide AAC systems for expressive and language disorders.
• Train clients, families, caregivers and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Demonstrate competent skills according to SLP standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate billing records and documentation.
• Respond to family requests and concerns.
• Coordinate training with Restorative Nursing and other care staff.
• Ensure resident safety and confidentiality and comply with laws.
