Job summary

Creative Solutions in Healthcare - West is seeking a full-time Speech-Language Pathologist to join our in-house team serving residents through person-centered care. You will evaluate swallowing and cognitive deficits, develop individualized treatment plans, and educate residents and caregivers to improve independence and functioning.

Active Texas license in Speech-Language Pathology is required, with SNF/long-term care experience preferred.

Qualifications

• Active Texas license in Speech-Language Pathology.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Ability to manage residents with different personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for severe expressive/language disorders.
• Train clients, families, and caregivers as subject matter experts.
• Coordinate with other professionals for diagnostic swallowing procedures.
• Maintain documentation and billing records.
• Observe confidentiality and protect patient information.
