Responsibilities

• Assess and document patient’s medical, mental and emotional needs at admission and ongoing; develop plan of care including discharge planning.
• Advocate for patients and families; ensure safe and timely care.
• Demonstrate clinical reasoning and perform patient care activities; evaluate therapy interventions.
• Communicate patient information and thoroughly document actions and plan of care.
• Provide clinical leadership and mentoring to staff.
