Job summary

Downtown Health & Rehabilitation Center in Texas is seeking a full-time Speech Language Pathologist (SLP) to join our in-house team serving residents through person-centered care and targeted therapies. The role includes evaluating swallowing and cognitive deficits, developing individualized plans, and educating residents and caregivers.

Responsibilities include AAC implementation, multidisciplinary collaboration for swallow studies, accurate clinical documentation, and training staff.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory agencies
• SNF/Long term care experience preferred
• Knowledge of current treatment practices
• Familiarity with Net Health Electronic Documentation System preferred
• Able to manage residents with different personalities

Responsibilities

• Evaluate residents' conditions for swallowing or cognitive deficits
• Assist residents in goals to improve cognition or swallowing
• Educate residents and caregivers
• Provide skilled, medically necessary speech and language therapy assessments
• Develop and implement individualized treatment plans to increase independence
• Provide AAC systems for severe expressive or language disorders
• Train clients, families, and staff as a subject matter expert
• Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES)
• Demonstrate competent technical skills according to current speech therapy standards
• Communicate changes in resident's condition
• Provide accurate billing records and supporting clinical documentation
• Respond to family requests and concerns timely
• Coordinate and provide training with Restorative Nursing and care staff
• Ensure protection of resident safety and confidentiality
• Comply with all laws and professional requirements
