Job summary

Creative Solutions In HealthCare seeks a Speech Language Pathologist (SLP) to join our in-house rehab team in Texas. The role supports residents with person-centered care to achieve positive outcomes.

Responsibilities include evaluating swallowing and cognitive deficits, developing individualized treatment plans, and providing AAC systems. Active Texas license and SNF/long-term care familiarity preferred. New grads welcome.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology
• In good standing with regulatory agencies
• SNF/Long term care experience preferred
• Knowledge of current treatment practices
• Ability to manage patients with different personalities

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents in improving cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and implement individualized treatment plans.
• Provide AAC systems for expressive/language disorders.
• Deliver training to clients, families, and caregivers.
• Coordinate with professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other staff.
• Ensure safety and confidentiality of patient information.
