Job summary

Care Center in Texas is seeking a full-time Speech Language Pathologist to evaluate and treat residents with swallowing and cognitive needs. You will provide assessments, develop individualized treatment plans, and educate residents and caregivers while coordinating with the care team.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments and develop/implement individualized treatment plans
• Provide augmentative and alternative communication (AAC) systems
• Provide training and education to clients, family, caregivers, and other professionals
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician
• Demonstrate competent technical skills per current Speech Therapy standards
• Communicate changes in resident’s condition
• Provide billing records and clinical documentation
• Respond to family requests timely
• Coordinate training with Restorative Nursing and other staff
• Ensure no resident abuse or neglect occurs
• Maintain confidentiality of patient information
• Comply with legal and professional requirements
