Job summary

Creative Solutions In HealthCare is seeking a full-time Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care to achieve positive outcomes.

Responsibilities include evaluating swallowing and cognitive deficits, developing individualized treatment plans, and providing AAC systems and caregiver education. Texas licensure and long-term care experience are preferred.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with all regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits for therapy need.
• Develop and implement individualized treatment plans to improve independence.
• Provide AAC systems for individuals with severe language disorders.
• Educate residents, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures.
• Maintain accurate billing records and clinical documentation.
• Ensure resident safety and confidentiality of information.
