Job summary

Responsibilities

• Evaluate residents to determine physical therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and staff in resident care activities.
• Communicate with physicians, nursing staff, families, and interdisciplinary team members regarding progress and discharge planning.
• Complete timely, accurate documentation meeting facility and payer requirements.
