Job summary

Gatesrehab is seeking a licensed Speech Language Pathologist (SLP) to provide skilled therapy services to residents in accordance with physician orders and care plans.

This PRN position emphasizes patient-centered care, collaboration with nursing and interdisciplinary teams, and clinical excellence in a long‑term care setting.

Qualifications

• Must hold CCC and state licensure for SLP.
• Graduation from an accredited SLP program.
• Experience in long-term care preferred.

This position is ideal for a therapist who values patient-centered care, collaboration, and clinical excellence.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide therapy services in accordance with physician orders
• Document evaluations, treatments, progress notes, and discharge summaries
• Collaborate with nursing and interdisciplinary teams

Qualifications

• Graduate of an accredited therapy program (SLP)
• Current state licensure in SLP
• Current Certification of Clinical Competence (CCC)
• Long-term care experience preferred
