Job summary

Casa of Hobart is seeking a compassionate Speech-Language Pathologist (SLP) to assess, diagnose, and treat residents with speech, language, cognitive, and swallowing disorders. You will work with residents, families, nursing staff, and an interdisciplinary team to develop personalized treatment plans that improve communication skills and quality of life.

State licensure and a Master’s in Speech-Language Pathology are required; CCC-SLP and CPR are preferred.

Qualifications

  • Master’s degree in Speech-Language Pathology.
  • Current state licensure as a Speech-Language Pathologist.
  • CCC-SLP certification preferred.
  • CPR certification preferred.
  • Experience in healthcare, rehabilitation, skilled nursing, assisted living, or pediatric settings preferred.
  • Strong communication and organizational skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Evaluate patients to determine speech, language, cognitive, and swallowing needs.
  • Develop and implement individualized treatment plans.
  • Provide speech therapy services per physician orders and care plans.
  • Monitor and document patient progress accurately and timely.
  • Educate patients, families, and caregivers about therapy goals and home exercises.
  • Collaborate with nursing staff, therapy team, physicians, and other departments.
  • Maintain compliance with state regulations, facility policies, and professional standards.
  • Participate in care conferences and interdisciplinary meetings as needed.
  • Maintain patient confidentiality and HIPAA compliance.
  • Complete required documentation and billing in a timely manner.

Speech Therapist / Speech-Language Pathologist (SLP)

We are seeking a compassionate and motivated Speech Therapist to assess, diagnose, and treat individuals with speech, language, communication, and swallowing disorders. The ideal candidate will work collaboratively with residents, families, nursing staff, and interdisciplinary team members to develop personalized treatment plans that improve quality of life and communication skills.

Essential Duties And Responsibilities
  • Evaluate patients/residents to determine speech, language, cognitive, and swallowing needs
  • Develop and implement individualized treatment plans
  • Provide speech therapy services in accordance with physician orders and care plans
  • Monitor and document patient progress accurately and timely
  • Educate patients, families, and caregivers regarding therapy goals and home exercises
  • Collaborate with nursing staff, therapy team, physicians, and other departments

Physical Requirements
  • Ability to stand, walk, bend, and assist patients during therapy sessions
  • Ability to lift up to 25 pounds occasionally
  • Ability to communicate effectively verbally and in writing
