Job summary

Heritage Manor Care is seeking a Speech-Language Pathologist to provide and supervise comprehensive patient care in a post-acute setting. You will screen, evaluate, treat, plan goals, educate families and document outcomes to achieve the best patient results.

You will advocate for patients, coordinate with multidisciplinary teams, and contribute to quality assurance and service excellence in a collaborative work environment.

Qualifications

• Master of Science in Speech/Language Pathology.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• CFY (Clinical Fellowship Year) candidates considered.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Ensures patient care follows guidelines and standards and participates in quality assurance and customer service activities.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation, in order to assure the best outcome possible for the patient. The Speech Language Pathologist (SLP) ensures that patient care follows the organization’s guidelines, professional standards and community needs, and is in compliance with Federal and State guidelines.
• Serves as a patient and rehabilitation advocate for current and prospective patients and for internal and external customers. Ensures cross-disciplinary coordination to provide for consistent patient care. Participates in quality assurance and customer service activities.
