Job summary

Scotlandmanorrehab is seeking a licensed Speech Language Pathologist (SLP) to provide skilled therapy services to residents in accordance with physician orders and care plans. This PRN role focuses on improving resident function, communication, and quality of life, within a collaborative rehab environment.

Qualifications

• Graduate of an accredited therapy program (SLP).
• Current state licensure in SLP.
• CCC Certification (ASHA) required.
• Long-term care experience preferred.
• Strong documentation and communication skills.
• Patient-focused and team-oriented.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide therapy services per physician orders
• Document evaluations, treatments, progress notes, and discharge summaries
• Collaborate with nursing and interdisciplinary teams
• Participate in care planning and utilization review meetings
• Educate residents, families, and staff on therapy goals and plans
• Maintain productivity and documentation standards
• Support regulatory compliance and survey readiness
