Job summary

Folkston Park Care and Rehab in Folkston, GA is seeking a PRN Speech-Language Pathologist (SLP) to join our long-term care/skilled nursing facility team. The role involves evaluating and treating residents with speech, language, cognitive-communication, and swallowing disorders, and developing individualized treatment plans.

The SLP will collaborate with interdisciplinary teams to ensure high-quality patient care and will document evaluations, treatment sessions, and progress using Net Health

Qualifications

• Master's degree in Speech-Language Pathology required.
• GA state licensure as a Speech-Language Pathologist required.
• ASHA CCC-SLP certification required.

We are seeking a compassionate and experienced PRN Speech-Language Pathologist (SLP) to join our dedicated team in a long-term care/skilled nursing facility setting. The ideal candidate will be passionate about helping patients improve their quality of life and meet all certification requirements.

• Collaborate with interdisciplinary teams to ensure high-quality patient care
• Document evaluations, treatment sessions, and progress using Net Health (Therapy EMR system)

Requirements

• Master's Degree in Speech-Language Pathology
• ASHA Certification (CCC-SLP) required
• GA state licensure as a Speech-Language Pathologist
• Previous experience in LTC/SNF setting preferred
• Proficiency with Net Health or similar electronic documentation systems is a plus
