Job summary

Clearway Health is seeking a Pharmacy Patient Liaison I to bridge patients and pharmacists in our specialty pharmacy space. You will support complex therapies, collaborate with clinical teams, and drive patient enrollment and retention with a focus on high-quality care and patient outcomes.

You will manage patient relationships, use EMR tools, and navigate coordination across providers to ensure a seamless continuum of care. A pharmacy technician license and 2+ years of experience are preferred.

Qualifications

• Requires a high school diploma or GED; pharmacy tech license and national certification preferred.
• Active and unrestricted Pharmacy Technician license in the applicable state is required.
• 2+ years of direct pharmacy experience.
• Strong English communication and customer service skills are essential.

Responsibilities

• Develop patient relationships and drive enrollment in Clearway Health Specialty Pharmacy programs.
• Enroll patients, follow up on leads, and generate new business through provider relationships.
• Provide outbound adherence support and medication management to ensure continuity of care.
• Maintain relationships with providers and care teams to ensure smooth patient transitions.
