Job summary

SUNY Upstate Medical University seeks a Specialty Pharmacy pharmacist to perform patient outreach assessments for appropriateness of specialty medication therapy and document the care provided within the Specialty Pharmacy Patient Management Program software, supporting clinical activities, metric reporting, compliance and quality improvement projects for specialty pharmacy accreditation.

This role will oversee program expansion, focus on developing relationships with clinic staff and specialty

Qualifications

• B.S. in Pharmacy or Pharm.D. and NYS Pharmacist license and registration, or eligible.
• Active CPR required by time of appointment.
• 3+ years specialty pharmacy experience with clinical knowledge and therapy management for Transplant, HIV, Oncology, Rheumatology, pulmonology, and others preferred.
• Experience with Epic, PGY1 with preference to PGY2 Specialty trained.

Responsibilities

• Perform patient outreach assessments for appropriateness of specialty medication therapy and document care in the Specialty Pharmacy Patient Management Program software.
• Oversee program expansion and develop relationships with clinic staff and specialty providers.
• Help build third party payer and manufacturer relations.
• Participate in all areas of dispensing, call center and clinic as needed or requested.
