SUNY Upstate Medical University in Syracuse, NY seeks a Specialty Pharmacy pharmacist to lead patient outreach assessments, determine appropriateness of specialty therapy, and document care in the Specialty Pharmacy Patient Management Program.

The role supports clinical activities, metric reporting, compliance and quality improvement for accreditation, builds relationships with clinic staff, third-party payers and manufacturers, and participates in dispensing, call center and clinic as needed.

Qualifications

• Requires B.S. in Pharmacy or Pharm.D. and NYS Pharmacist license/eligibility.
• Active CPR required by time of appointment.

Responsabilités

• Document care in the Specialty Pharmacy Patient Management Program software.
