Job summary

CVS Health in Minnesota seeks a Pharmacist - Specialty Compounding to support home infusion patients. You will review charts and lab results for an assigned caseload, communicate with patients and caregivers, and discuss updates with physicians.

This role requires a Minnesota license, a PharmD or BS in Pharmacy, and willingness to obtain additional licenses. Experience in IV home infusion and CPR certification are preferred; strong clinical and communication skills are essential.

Qualifications

• Pharmacy degree (PharmD or BS) from an accredited college.
• Current Minnesota pharmacy license.
• Willingness to obtain additional licenses as needed.
• CPR certification if in direct patient contact.
• Experience in IV/home infusion is preferred.

Responsibilities

• Build one-on-one patient relationships during infusion therapies.
• Review orders for therapeutic appropriateness and safety.
• Interpret lab results and communicate with physicians.
• Supervise pharmacy technicians in compounding and labeling.
• Assist in training and performance improvement activities.
• Support remote patients by phone as needed.
