Job summary

Fairview Health Services in Shoreview, MN is seeking a Pharmacist to assist the pharmacy manager, provide clinical care, and supervise support staff. The role focuses on optimizing drug therapy, counseling patients, and ensuring compliant dispensing in a home infusion setting.

On-site responsibilities include collaboration with healthcare teams. Requirements include a PharmD/BS-Pharmacy, Minnesota license, and 1 year of practice experience.

Qualifications

High School or GED
B.S./B.A. or Pharm D in Pharmacy from an accredited school of pharmacy
1 year experience as a Practicing Pharmacist
MN Pharmacy License

Responsibilities

Perform initial patient evaluation for prescribed therapies and collect/analyze data to optimize drug therapy.
Evaluate signs and symptoms to modify drug therapy.
Monitor for drug interactions and allergies in patient records.
Provide counseling to patients, families, and healthcare professionals about drug therapy.
Dispense medications accurately with proper labeling and timely processing.
Assist with Home Care and Pharmacy billing processes.
