Job summary

InfuCare Rx is seeking a Specialty Pharmacist to oversee Infusion Suite operations, coordination with patients, caregivers and providers, and to support home infusion patients. You will interact with insurers, staff, distributors and physicians to ensure safe, high-quality drug therapy.

The role involves clinical and distributive pharmacy activities, documentation, and participation in QA and on-call rotations as needed. A PharmD or equivalent license is required.

Qualifications

• Two years’ recent experience in home infusion therapy.
• Ability to collaborate with physicians, nurses and other healthcare professionals.
• Strong communication and documentation skills.

Responsibilities

• Adheres to organization policies and procedures.
• Reviews and manages specialty medication therapies for complex, high-cost conditions.
• Coordinates and facilitates new referrals/admissions with nursing, customer service, and distribution.
• Reviews, receives/verifies all new patient orders and documents on the order form.
• Assists the physician and other healthcare professionals in planning and implementing patient care in the home/suite.
