Job summary

Community Bridges, Inc. in Arizona is seeking a Specialty Case Manager to provide community-based case management for individuals with serious mental illness or high-acuity conditions.

You will plan, connect, advocate, coordinate, and monitor services to help clients access health care, housing, financial assistance, and social supports. You will assess needs, develop individualized treatment plans, document services, and support a recovery-focused environment.

Qualifications

• Highschool diploma or GED required.
• Associates Degree or higher in a field related to behavioral health preferred.
• Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred.

Responsibilities

• Provide community-based case management for patients with serious mental illness or high acuity.
• Assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports.
• Assess needs, develop and monitor individualized treatment plans, and document services accurately and timely.
