Job summary

WelbeHealth PACE in Rosemead, CA is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) who is Spanish-speaking to deliver comprehensive primary care to frail seniors in their homes and communities.

As part of an interdisciplinary team, you will perform in-person assessments, develop care plans with physicians, and support discharge planning. Competitive compensation with quarterly bonus and strong benefits are offered.

Qualifications

• Certification as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Experience caring for frail or elderly populations is preferred.
• Graduation from an accredited nursing program and current licensure.

Responsibilities

• Provide continuous medical coverage and conduct comprehensive in-person assessments.
• Collaborate with IDT to develop care plans and coordinate care.
• Assist in discharge planning from acute or long-term settings and monitor outcomes.
