Job summary

Neighborhood Healthcare in Riverside, CA, seeks a compassionate Nurse Practitioner or Physician Assistant to join the PACE program as a Primary Care Provider. You will deliver participant-centered care within an interdisciplinary team, supporting health outcomes and quality of life.

Schedule: Monday–Friday 8am–5pm, with opportunities to lead and mentor others while maintaining comprehensive documentation in the EHR and coordinating with home care teams.

Qualifications

• Master’s degree in nursing or PA program and licenses as required by role.
• California NP or PA licensure in good standing with DEA where applicable.
• BLS and CPR/First Aid certifications required and renewed annually.
• Board certification or eligibility within 12 months of hire.
• Experience in geriatrics, chronic disease management, or home-based care preferred.

Responsibilities

• Provide comprehensive primary care to PACE participants in a hospital/clinic setting.
• Perform exams, order and interpret diagnostics, and manage acute/chronic conditions.
• Deliver injections, immunizations, wound care, and preventive services.
• Coordinate with an interdisciplinary team to plan and revise care plans.
• Document care plans accurately in the EHR and ensure continuity of care.
• Conduct home visits and educate participants and families on therapies and lifestyle.
• Mentor other team members and contribute to quality improvement initiatives.
