Job summary

Arbour Counseling Services, a subsidiary of Universal Health Services, is hiring a Community Support Program Case Manager for full-time, in-person work. The role focuses on adults 18+ with mental health needs, providing outreach, coaching, and connection to resources to support independent living and reduce hospitalization risk.

Responsibilities

Meet client weekly to provide outreach and support services
Engage client in the treatment system and support them to implement their goals
Support client to attain skills and resources needed to maintain community tenure.
Coordinate Care with the providers/contacts
Provide telephone coaching and support
Complete documentation of all meetings and contacts
Attend weekly staff/department meetings and supervision
