Universal Health Services’ Arbour Counseling Services is hiring a Community Support Program Case Manager in Worcester, MA. The role focuses on outreach, treatment engagement, and community-based support for adults with mental health needs, offering flexibility, growth, and a strong benefits package.

We provide skills training, resource connections, and supervision in a supportive team environment to reduce hospitalization risk and improve quality of life.

Qualifications

• Bachelor's degree preferred in Psychology, Human Services, or related field.
• A valid Massachusetts driver's license.
• Daily access to a working, registered and insured vehicle.
• Excellent in both written and verbal communications.
• Ability to independently plan and organize one's own schedule and client services.
• Knowledge of Microsoft computer systems.
• Strong time management and organizational skills.
• Bilingual applicants encouraged.
• Citizenship/work documentation required.
• CORI and drug test and other pre-employment documentation required.

Responsibilities

• Meet client weekly to provide outreach and support services.
• Engage client in the treatment system and support them to implement their goals.
• Support client to attain skills and resources needed to maintain community tenure.
• Coordinate care with the providers/contacts.
• Provide telephone coaching and support.
• Complete documentation of all meetings and contacts.
• Attend weekly staff/department meetings and supervision.

Our Community Support Program Case Managers work with adults 18+ in the community, living with all types of mental health needs. We provide skills training and connections to community resources to support independent living in their communities, decrease the risk of hospitalization or relapse of substance use, and improve overall quality of life.
