Job summary

Arbour Counseling Services, a part of Universal Health Services, is seeking full-time Community Support Program Case Managers to work with adults 18+ in the community. This role offers a flexible schedule, a supportive environment, and competitive compensation including a signing bonus.

You will provide outreach, help clients engage with treatment services, and coordinate care while maintaining detailed documentation. Training and licensing support are provided as part of growth opportunities.

Qualifications

• Bachelor's degree preferred in Psychology, Human Services, or a related field with one year of direct care experience.
• Massachusetts driver's license is required.
• Proficient in Microsoft Office and basic computer literacy.

Responsibilities

• Meet client weekly to provide outreach and support services.
• Engage client in the treatment system and support them to implement their goals.
• Support client to attain skills and resources needed to maintain community tenure.
• Coordinate care with the providers/contacts.
• Provide telephone coaching and support.
• Complete documentation of all meetings and contacts.
• Attend weekly staff/department meetings and supervision.
