Job summary

Ascension Saint Agnes Hospital in Baltimore, MD seeks a Social Worker for the Social Services department. This day-shift, full-time role focuses on assessing patients’ social needs and coordinating post-discharge plans to ensure smooth transitions.

You will work with a multidisciplinary team to connect patients and families to essential resources, ensuring compliant, compassionate care throughout the treatment journey.

Qualifications

How you’ll make an impact in this role

• Guide patient care journeys by assessing social, emotional, and environmental needs to build compassionate, personalized intervention plans.
• Coordinate seamless post-discharge transitions through active involvement in concurrent reviews, discharge planning, home care, and continuing social services.
• Connect patients and families to critical resources by collaborating with community agencies, healthcare partners, and support networks to secure vital assistance.
• Align care strategies with multidisciplinary teams by consulting with clinical managers, external organizations, and loved ones to ensure complete treatment compliance.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

Required Credential(s)

• Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date. Any level accepted.

Preferred Credential(s)

• Drivers License credentialed from the Maryland Department of Motor Vehicles.

Education

• Required: Bachelor's Degree in Social Work at date of hire.

Additional Preferences

• Schedule: Monday-Friday 8:00AM-4:30PM, no weekends or holidays. 1 remote day after 6 months of employment.
