Qualifications

Responsibilities

• Performs psychosocial assessments of patients and families in the hospice program.
• Counsels patients and family caregivers; coordinates with the clinical team.
• Documents services accurately to reflect the plan of care.
• Assists with applications for SSI, Hospice Medicaid, and Medicaid Waiver.
• Acts as liaison with community social health and welfare agencies.

Qualifications

Social Work DegreeTennessee Masters Degree in Social Work requiredCurrent State Licensure in Social WorkTennessee LMSW or LCSW requiredMinimum of one years experience in health care or hospice settingValid drivers license and proof of insurance is required

Performs the psychosocial assessment of patients and familiescaregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patients plan of care Provides an assessment in the patients identified residence and assistance when this is not safe and another plan is requiredDemonstrates exceptional service to patients families referral sources co workers and other customers Assist team members as needed to ensure that this level of service is consistently providedCounsels patient and family caregivers and staff as needed in relationship to stress and other identified coping difficultiesProvides accurate and timely documentation of patient services to reflect the plan of careProvides assistance with applications for Supplemental Security Income Hospice Medicaid and Medicaid Waiver Obtains resources from community social health pharmaceutical and welfare agencies to assist in meeting patientfamily needsServes as liaison between patients and familiescaregivers and community agenciesReady to Start a Rewarding Career
